Activity Report

Students enrolled in research credits or Maintenance of Matriculation are required to complete the Doctoral Research Activity Report(link is external) at the end of the semester and submit to his or her advisory committee. The form should be signed by the dissertation advisor and submitted to the Office of Graduate Academics. Students who do not submit the Research Activity Report at the end of the semester will receive an academic hold on their account and as a result will not be able to enroll in additional credits.
